Physician Computer Access Request

If you do not have a username and password, you must obtain them by submitting a Computer Access Request form which you can download and fax in.

Step by step:
  1. Download Computer Access Form
  2. Download UHS Confidentiality Form
  3. Fill out (the tab key will get you from field to field)
  4. Save file and print
  5. Sign forms and fax to (702) 853-8953
